You will receive care from our fully licensed/certified staff that have been screened and evaluated for professional competence and character. We will treat you with courtesy and respect and all information will be kept confidential. You will be told the name and classification of all personnel caring for you, as well as participating in planning your care. You will receive information about your diagnosis, prognosis, treatment, and medications prescribed in consultation with your physician. While receiving our care, you will have the right to make decisions and to refuse or accept treatment and you will be told in advance the charges for services and the amount you will be responsible for. This is our commitment to you.

Myrtue Medical Center Home & Public Health/Hospice Department:
- Is a Medicare/Medicaid approved home health agency.
- Our home health team can provide intermittent services to meet your home care needs.
- Call us at (712) 755-4308 when:
- Your physician recommends home health care.
- You have a health condition that requires medical or nursing care.
- You have a health condition that limits your daily activities and you need help
with dressing, bathing, and other personal cares.
- A nurse is on call 7 days per week - 24 hours per day.
- All client care is supervised by a registered nurse who coordinated the development of your care plan. The health care team works with your physician and family to develop a plan appropriate to meet your needs.
- Services are available to persons residing or temporarily living in Shelby County and surrounding areas.
- A Registered Nurse skilled in home care is available for home visits.
- Skilled Nursing Care includes, but is not limited to:
- Coordination of care with physician
- Patient and family teaching
- Diet teaching, nutritional instruction
- Wound care, dressing changes
- I.M. Injections, medication administration and teaching
- Medication set up and use of medication dispenser
- Skilled observation and health assessments
- Blood draws for lab tests
- IV Therapy
- Pain management
- Catheter and ostomy care and teaching
- Assistance with problem solving in home care
- Psychiatric nursing
- Referrals for other community services
- Physical, Occupational, and Speech Therapist
- Medical Social Worker
- Enterostomal, Wound Specialist
- State Certified Home Care Aides who are available for:
- Assistance with personal hygiene
- Assistance with bathing, and dressing
- Light housekeeping and laundry
- Meal preparation
- Assistance with simple exercise, transfers or ambulation
- Shopping for groceries
- Child protective services
- Assistance with budgeting and parenting
